This company, Universal Service Administrative Co., mailed my mother some paperwork titled "Lifeline Program Application Notice" with an "Important Message about your Lifeline Phone Benefit." The packet of information is addressed to my mother using my mother's first name, someone else's last name (unfamiliar, do not recognize) and my mother's correct adress. The letter appears to be somewhat official and was dated for February 25, 2021. The letter begins: "Dear _____, You recently applied for the lifeline benefit with a phone/internet company. Lifeline is a federal program that lowers the monthly cost of phone and internet for qualified consumers. We need more information from you to approve your lifeline application. To qualify, we will need you to submit your documents by March 29, 2021. STEP 1: Find and prepare the following items: A) Proof of eligibility (We rejected your previous document) and B) Proof of Social Security number (We rejected your previous document). STEP 2: Make Copies of your item. The copies of pictures you provide must be clear and easy to read. We cannot process dark or blurry images. Please never submit original copies of your documents. STEP 3: Submit your copies for review. You can complete your application in three ways: 1) call the phone/internet company you applied with to help you complete your lifeline application. 2) Go online to www.CheckLifeline.org and upload your documents to your application. 3) Mail your copies and the page titled Cover Sheet included with this letter to: Lifeline Support Center, PO Box 7081, London, KY 40742. This letter seems like it is legit because at the bottom of the letter there is a footer that reads: WARNING: Lifeline is a federal benefit programthat is available to qualifying consumeers. Willingly giving false or fraudulent information to get Lifeline Program Benefits is punishable by law and can result in fines, jail time, de-enrollment, or being barred from the program. *Only the last four (4) digits of the consumer's Social Security number is used to verify the consumer's identity.

I first took the time to look up their website, and just like in their letter they tout themselves to be a federal program. I called the number on the letter and raised the issue that I think someone else may have applied for benefits under my mother's first name and address with the wrong last name. She asked me for the full social security number or my address to look up the information. I am afraid an Identity theft may have occured and I didn't want to give them my mother's personal information. The woman told me that I could just disregard the letter. I didn't feel confident with that answer so I ended up hanging up and I looked them up on the ScamPulse.com site and found that they are not accredited by the BBB. They are asking people to send/give up sensitive identifying information and claiming to be a federal program. I just don't trust this business has any valid federal, or US Government affiliations and is either milling for sensitive information or someone has used my mother's information to apply.
