I filled out the form including my credit card for payment. They promised expedited shipping and when I was done it brought me to a preprinted form to fill out and mail to the vital records office in which I should include my payment! I just paid with my credit card. They took $90 for 2 copies and basically sent me a preprinted form, that's it! There is no customer service number. Just a contact form. I also got an email saying my order was complete. Ive emailed them twice asking for a refund and have heard nothing. DONT FALL FOR THIS SCAM!! ORDER DIRECTLY FROM THE GOVERNMENT WEBSITE OF THE STATE YOU WERE BORN
