Sending back brace. They knew my doctors name. And I did have an X-ray on my back. And an MRI scheduled so it made sense to me. They had my address but then asked to verify address when setting up to send by UPS. Never asked for insurance companies or insurance number never asked for SS#. Mainly name and address for mailing brace in 5-7 days
