A envelope was sent with contents of: (1) a "Cardiovascular Disease (CVD) Genetic Requisition Form" from Infinity Diagnostic Laboratory - address of 370 North Street, Teterboro NJ 07608, an oral swab kit and a return FedEx envelope. The form had already entered information of my friend's name, date of birth, address and phone# as well as her doctors name, address, phone, fax and NPI #. It also had the name of her insurance and her ID# for that insurance. Her doctor's signature was verified by her though the doctor stated she had not signed the form or requested the test. The form also asked for a copy of my friend's insurance card or other payment (e.g., credit card) and her signature.
