They sent a letter to my 91 yr-old father telling him his bill was past due for Accidental Death & Dismemberment policy, providing Company ID, insurance amount, quarterly amt due of $9.90, carrier - TransAmerica. They only wanted the form to be completed with either Credit Card or Checking Account information. My father never inquired or had a policy for this type.

We called and was put through to "Customer Service Rep" after following automated info. Got live person who would not identify self or company they worked for. Conversation went this way: "Customer Service Center"; we asked "Who do you represent?; She stated again" Customer Service Center. This was done 3 times until she stated that she could not hear and she would have to disconnect the call. All the time responded to questioning with statement "Customer Service Center" but refused to identify self each time.
