On 1/17/2023 I received two postage paid cards from Blountstown,FL. Permit No.4 from Agency For Health Care Administration Notice of Privacy Practices. One card has my ex husbands name Nestor Cintron with my address, and the other card has my sister name Emily Vasquez with my address, neither persons have ever resided in my home. And one entity I'm in litigation with, which has caused me distress seeing his name arriving on mail, at my mail box I called to see if this was some weird mistake 1-877-254-1055, when I dialed the phone hangs up on me, I tried the website number and get same results.
