I had been interviewing for Health Coach jobs on LinkedIn. I was contacted via email “HC” and told my application and credentials looked good. I was told to use Teams and contact Alexa Mihalec at [email protected]. Completed an interview via Teams Chat. Told Juan Rodriguez VP would be calling me re: interview. On 3/29/24 he called from 917-727-9635, conducted a short interview then offered me a remote job. I then got forms via Teams, all with realistic Form Health logo, including W-2, direct deposit, and enrollment form from alexamihalec@forumheslthworks, still via Teams chat. I completed them and sent them back to her, then was told my Team lead would be contacting me next day. I then was contacted (still Teams chat) by a Greg Worthum, who asked me to purchase an IPhone and IPad which I would be reimbursed for. I sent him back the receipt for same. Then he started asking me for $2000 for software. I refused and he began getting snippy. Eventually he said he’d send me a check via Teams for mobile deposit. My bank refused it. He then said he’d send me a check, which was FedEx overnighted to me for $9630 for reimbursement plus money to pay vendor for software. It was drawn on Bank of America. FedEx return information was Candy Guilliano, Carlson Company,

4333 S 86th E Ave, Tulsa, Ok.

I took it to my bank, told them I suspected a scam, and they advised me not to do anything with it. I called Forum Health to alert them, and someone called Alexa Mihalec from HR returned my call and told me she never heard of me, nor Greg Worthum, and that she used email [email protected]. I made an FTC complaint, closed my bank account, froze my ss account and froze my credit. I will not respond to Greg Worthum again.
