I received a W-2 form in the mail from this company, and I have never heard of them, worked for them, or been in the state of California. They have indicated having withheld Social Security taxes, Medicare taxes, and have the earnings listed under CA state wages, tips, etc. They've got my correct address and social security number. There's an employer's federal I.D. number and a verification code.

I looked up the address, and it is listed online as a female attorney's address. The only American Council of Engineering I could find that covered the San Jose area was in Sacramento.
