It didn't provide me a company name, its address, URL or even a telephone number; so, in your scam questionnaire, I provided the UPS 800 number. The tracking code for the package is: US-1RA-5NAMCM.
As I've never ordered anything online for this amount and quite frankly wouldn't because my porch is an insecure space for packages, this email and query is a scam, I hope you can investigate it. Hence I've filled out this form. Please contact whether or if you need more information to research this alleged scam.

Ps: your onlline scam questionnaire is isn't user friendly and several times I had to fill out the forme because every time I toggled between screens, the form would blip off my phone. Grr.
