I filled out the form 2 weeks ago and received a package from 731 Mckinnon Ave, San Francisco, CA 94124-2140. Inside included a cashier's check from Star One Credit Union for $2,450 with instructions to go to one of the locations on the paper and purchase an eBay gift card with denominations from $100, $200, and $500 to equal $2000 total. Once you purchase the gift cards the instructions explain to peel the silver scratch-off area, capture an image of the card front and back, and email [email protected] your customer experience and images of the eBay gift cards.
