Presented an invoice from the CT UCC Statement Service. Used a bank's name that I have a loan with (BMO Bank Na). Had my name and mailing address. Requested that I send $98, review and complete a form. Then detach and send the form with a check or money order in the envelope provide. That mailing address was indicated earlier as: 341 E Center Street #141; Manchester, CT 06040.

When I searched for the address it looks like a private mail store. Guessing someone is collecting checks in a mailbox from less savvy people.

Additional information requested on the detachable form: Name, email and phone number. Asked for a signature and date.
