The letter sent was formatted to mimic an official government form. Though it is stated in the text that the scammer is not a government entity, it would likely fool those not paying close enough attention. The envelope has no return address but states it was mailed from zip code 34102.

Upon forming a new LLC I reviewed this fake letter demanding $295 for a ******** Safety Manual or face fines up to $70,000. Total scam and I received multiple letters from this same company requiring different fake services. Please shut them down and prosecute. Letter attached.

Received a form in the mail with all of the information for my LLC asking me to send $295 for Federal OSHA laws required safety manual. This is the 3rd time I have received this letter.

In addition to the info it included about my LLC (Business ID #, business name, & address), it includes a bar code, and the following numbers: ND, CID

I have received this scam letter 3x's now and finally decided to take action. Companies like this should be shut down immediately.
