Received from Senior Information Dept., but has the exact same mail info though the city name has changed. See pict. Now it is a survey and they state you will receive a free WalMart gift card 4-6 was after the completed survey is returned. No stated amount of gift card and they want name, spouse, address (no P.O. Boxes), city, state, zip, phone, and best time to call. Has 8 questions all relating to insurance.
