We are a specialist doctor office in Chattanooga, TN. Elite Medical Supplies sent a fax to our office requesting pertinent chart notes and an Rx form. Their fax was for a lumbar-sacral, knee, and shoulder/elbow/wrist/hand orthoses that none of our physicians ordered for this patient, nor did the patient request these themselves. The patient's name, DOB, address, phone number, and medical policy number were all listed on this form.
