I'm sorry if you feel that you've been inconvenienced by the language of the website. Since you are the first person to have been confused about it in over 6 years I think we're going to leave it the way it is.
An ESA letter is like a medical prescription...it is valid for a period of one year. Airline forms are written based on the last valid assessment made and are always dated on that date.
Since your letter was written over 5 years ago there is no way that a form can be written based on it.
As an accommodation, we will offer to do a new assessment at our renewal rate of $95. Should you choose to do this it will be necessary for you to submit a new Evaluation form which can be found at the following link and we will invoice you for the balance of $75:

This is obviously confusing, and indirectly calling me dumb by saying I’m the “first person to be confused in over 6 years” so you won’t change the blatantly contradicting verbiage is highly offensive.
You specifically said “additional forms requested at any time after the letter was written will incur an admin fee of $25...” and you’re saying that after five years it’s $95? This is 1000% contradictory on your website. I don’t care the reasoning you have about prescription or anything, if there is a conditional term it needs to be corrected on the website, maybe consider it seriously if you want to maintain a very basic level of professionalism in the company.
Please don’t continue to undermine and be condescending toward customers, instead maybe consider a career outside of customer service?
