I decided to succumb to the threatening letter to call but with a plan to not give out any vital personal info if they should ask. Sure enough, when you call the number they immediately want personal information (name/birth date/etc) from you. I just let them know that if they have access to the doctor visit and/or Blue Shield they should have all that information about the visit and there is nothing else I would add nor would I be giving out any of my personal information. He then says okay that he doesn't have anything else to ask me. Which makes the whole call and the "extreme urgent" letters seem like phishing scam to attempt to pull personal info verbally from me. I don't believe this company has a legit purpose for anything on behalf of Blue Shield Blue Cross.

Upon further inspection, there were many other inconsistencies with their claim. The return address on the Fedex envelope I received the check in (as it was overnighted to me, received on 6/15/16) lists an address in Washington state. Said address on Google shows a parking lot of a community college. The information from the tracking number that Fedex gave also stated that the package was initially picked up in Miami, FL, from a business located in Indianapolis, IN (Equian LLC). The location listed for the check itself was for a construction company in Iowa.
On top of all of this, Mr. Daniel Gibson often spoke in a less than perfect English, with most of what he said feeling prewritten as part of the scam. The letter I received with the check also spoke in not fantastic English, but was from a Mr. Michael Rodgers, a man whose name was previously not mentioned in any conversation I had with Mr. Gibson.
As well as the incomplete English, some areas of the letter aren't consistent in terms of writing, with the "Rodgers" portion of Mr. Rodgers' name being written in a separate font. There is also no company letterhead or any appearance of professionalism in the letter, something that could have easily been written by anyone with access to Microsoft Word.
