The credit card section asks for card number and expiration.
It lists three address to choose to take this course at; Houston, DFW and San Antonio. It also mentions that these address and dates will be confirmed upon receiving payment.
The only appealing thing about this form is the fact that "Lunch and Refreshments" are included. Plus I haven't been to the River Walk in San Antonio in a while, so that would be fun.
The course states that it is $425.00 per person. Typical "to be removed from our list please call 1-888-497-4557" and instructions to visit www.osha.gov and search "forklift"
