Periodically for the past several years an electronic check has been presented against my account. The funds are debited and deposited to an account with Lakeside Bank, Chicago IL #5000093624 with "FOR DEPOSIT ONLY REM DEP CAP" stamped in signature area. Also there is a customer service number provided on the front and back of check 877-873-0457, which when called asked the caller to leave a name and number as all customer service representatives are busy. In the past, I have caught the check before payment was transferred. This time, I was not quick enough and $495.00 was stolen.
