Recieved a letter in the mail saying we needed to fill out a Safety Manual Order Form for our newly created business saying that if we didn't complete this form we would be in violation of OSHA and to pay it to paysafety.org, or the e-mail or mailing address given earlier. Also threatens that fines and/or civil actions would be imposed for failing to fill out the form.
