Our office received a fax from Liberty Medical Supplies with a patient's legitimate Medicare ID# and was asked to sign a prescription request and letter of medial necessity for supposedly ordered orthoses. It looks like they are changing their business name but the fax number stays the same. The phone number area code is from Florida (470) and one fax number area code is Los Angeles (203). They are extremely persistent and have called our office several times looking for this information and the patient reported that they also called her primary care doctor looking to get this script signed so they can fraudulently bill Medicare for DME.
