AFFIDAVIT OF FACTS
I, ******* * ***** ** being duly sworn on oath, state that I have personal knowledge of the
facts contained herein, that the answers are true and correct to the best of my knowledge and
belief and, if called as a witness, that I would testify as follows:
On December 18, 2023, I contacted Ayo Odimayo, who is the listed manager for the Illinois
business Moniasons Surplus, by phone and email about a Vasomedical TS3 EECP table
that was listed by Dotmed.com and indicated my interest in purchasing the table.
2) The business address for Moniasons Surplus (Moniasons, LLC) is listed as: 311 E
WALNUT STREET, CHATHAM, IL 62629.
On December 22, 2023 an invoice was sent by Moniasons with a total amount due of
$7185.00.
This included the $6500.00 cost of the table and $685.00 shipping from Texas.

On December 26, 2023, check number 5269 was drafted for the above amount and sent
via USPS to the address listed on the invoice: 7011 PRESTON DRIVE, SPRINGFIELD, IL
62711.
On January 8, 2024 I sent the delivery address and instructions to Moniasons and received
the reply that the aforementioned check would clear their bank on Jan. 9th.

In a reply email on January 11th, I was informed that their technician would inspect the
table before inspection and that it would be created and shipped out tomorrow (Jan. 12th).

On Jan 18th I was informed via email that the technician had found an issue and that if it
was not repairable I would be issued a refund.
8) My response was that I was interested in the table at a discounted rate if it could not be
repaired.

After more email exchanges I was informed on Jan. 30 that if the table could not be
repaired and shipped by Thursday (Feb 1) I would be issued a refund.
10) On Feb. 1, I issued a complaint with DoMed concerning their clients possible breach of
the Domed code of ethics. Domed contacted Moniasons and on Feb. 9 terminated the
account on Domed with Moniasons with the following email repose:
"Ayo Moniasons registered on DOTed with bogus information and therefore probably has
something to hide. We have banned the user from our site and are sending you this
message because you were contacted by this user through our site.
We regret any inconvenience this may have caused you.
11) On Feb. 11, I again contacted Ayo and requested my refund by Friday 16.
12) As of the date listed below, I still have not received any refund on my purchase or shipping
cost.
