I forwarded a copy of the letter to my accountant and they verified it is a scam. The address to mail the check they are requesting is a UPS store in Oklahoma City and the suite number is a box they have at the UPS store. I called the number on the form and the man who answered said it was a legitimate business and it told him I would be making a formal complaint and he said ok and hung up on me.
