I attempted to download a DS-11 passport form and somehow ended up on ‘myusatraveldocs.org’. I was charged $108.65, mistakenly believing I was paying passport fees to the U.S. Department of State. This site misled me into providing my personal information, posing as a legitimate government agency with a deceptive letterhead stating “U.S. TRAVEL ONLINE APPLICATION” alongside a photo of the U.S. Capitol Rotunda. Their legal disclaimer, in very small print at the bottom of the page, reveals that “myusatraveldocs.org is a private business, not an official government organization nor affiliated with any U.S. government agencies.”

I was scammed out of $108.65, thinking they would submit my application on my behalf. Instead, they simply allowed me to enter my information on their website and then provided a PDF for me to print. Ultimately, I ended up doing the same work I would have done on the official government website. Now, I have to mail this document to the government agency and pay an additional $225. They did nothing but send me a PDF version, which is a scam. I did all the work!
