When I searched for "state of oregon vital records", this website was the second hit. (I've included a picture in the attachment)

I read the description and believed that I was ORDERING a copy of my daughter's birth certificate.
Instead, the company charged me $47 for just the APPLICATION and then expected me to submit the form AND send additional payment to the Oregon Vital Records department. I sent my application to the Oregon Vital Records and received it back stating that the payment was not received. I was still confused at that point. My credit card was charged the $47 plus a $0.38 "international service assessment" fee. My bank even sent a text message per their fraud alert department. I tried to cancel the transaction at that time but since I had made the purchase they didn't stop the transaction.
I have sent an email to the Health Policy Board to make them aware of the situation.
I also spent time with my bank to dispute the charge.
