this fake medical device company faxed my office requesting us to fill out a form they provided with many typos asking for patient information - they already had patient information filled out on the form and were asking for money. they provided only their number and fax number no website or address of their company. when you look up the name of their company there are other scams surrounding it. when we called the number, the person did not give us their name over the phone but they did sound like they were working at a large call center which makes me believe this is a large scam.
