I did not!!! I requested a copy of the form the office received and it is a form for my physician to sign off on this monitor. They received the form by fax.
I called Orthomedix Medical Supply at the number noted on the fax: 561-396-9571 (fax 561-359-0725) Was connected to someone and asked how they go my information and who told requested this BP monitor. They claimed someone spoke to me and I requested the item. Asked him to whom I spoke on his end and he said he would look. Within one minute the phone was hung up. I have tried calling back twice and both times the phone only goes to voice mail. This is of great concern as they have my personal information (date of birth, medicare number etc.).
