Type of a scam Phishing
After some extensive research, I learned that other individuals, around the nation, received the exact same letter, but from a different sender/company using the same return address as I show above. The only slight difference between my letter, and others, is in the paragraph with the heading of "What Happened?" Also, copies of these letters, to other individuals, are on various state attorney general's websites.

I don't know if this is legit or not, but it seems suspicious none the less.
