This was the email I received offering me the job:

Hi *** I want to thank you for your dedication and enthusiasm during our interview process. I really enjoyed our conversation and was impressed with your professional experiences and passion.

After careful consideration by our team, we are excited to offer you a position as a contract therapist (1099) at the Sleep and Counseling Center.

To get started, please fill out our onboarding form, which you can access here.

The form has three parts, and you can save your progress anytime by clicking the save button at the bottom.

As a clinic operating across multiple states, we welcome you to upload any additional state licenses you hold. We are pleased to assist you in getting set up for those states.

Upon form completion, we will initiate the credentialing and contracting process to ensure your inclusion in insurance networks and your smooth transition as a clinician at our practice.

Here's a summary of what to expect next:

1. Onboarding – before the end of next Friday, June 21: Please fill out the onboarding form to start the credentialing & contracting process.

2. Credentialing & contracting – next 90 days: Once you submit the onboarding form, our support team will be in charge of the credentialing process, which typically takes around 90 days. We'll keep you updated on any changes.

3. Training – 4hr at your pace: After credentialing, please complete our sleep training curriculum (about 4 hours of material) at your own pace. Then, a team member will help you set up our EHR system and guide you through its use.

4. Seeing clients – within 2-3 weeks: You can start seeing clients within 2-3 weeks after completing step 3.

At the Sleep and Counseling Center, we value our clinicians and aim to keep you well-informed throughout the onboarding process with regular updates. Our team is also available to provide guidance and answer any questions you may have.

I have not heard from the above person. I have attempted to email Donna, no response. Our last interaction was back in July during a video interview. I'm very worried because of the information I provided during the on-boarding process. I provided my license # (I'm a licensed clinical social worker), my malpractice info, copy of diploma and NPI number, copy of license (I believe) and address/phone #.
