Address was listed as 3601 Churchview Drive, Gibson, FL, 33544. Enclosed with the letter was a check for $4100.10 and instructions to contact Shahnaz Khan, Claim Agent, at 647-784-0854, to activate the check. Processing fees would be $2940.00. Letter signed by Annastezier Hinds, Promotion Manager.
