An unsolicited saliva kit arrived with a pink return envelope. The medicare policy number already filled in. The return address said ABCO Health and Wellness at 2310 SE Second St, Suite 7 in Boynton Beach, FL 33435, but the pic on google maps looks like a small store front that was empty a few months ago.

Then the paperwork shows the lab is at R&R Labs at 600 Fairway Dr Suite 104, Deerfield Beach, FL 33441. But the Ordering Physician is Rashad Awan at Telesolutions MD PC, 284 Dupont St Suite 130, Corona, CA 92879. First this suite doesn't exist. Second, this Dr doesn't exist. Third this company does not exist.

There is nothing about this that looks legit.

My elderly neighbor received a box of medical supplies that she did not order. It was three wrist braces and two hip braces. She asked her doctor as well as her physical therapist and they did not order them for her. Medicare told me to contact the companies, the one who delivered it and the one who was billing it from Texas which was Firstcare Medical Supply LLC. One phone was disconnected and the other did not mention a company name, only a man's voice that said to leave a message. Medicare is escalating it to fraud since the order was sent to them to pay for the supplies.

Said our Medicaid cards were no longer working and they needed to replace them with plastic cards. I gave them our numbers. I did report this to Medicare. They also sent me a genetic testing kit to find immunodefiency. I am suppose to swab my mouth and return it. The laboratory is called Prince Laboratories. The testing came with a card for Living Well Screening.

At least 20 phone calls a day from various scammers. This unified portal knows my Moms doctors name and often state the dr prescribed these braces (the doctor did not) They ship these braces to the house and ups and fix ex just drops them and will not send them back to sender. Calls to AVA Med Supply led me to Firstcare Medical Supply, 4606 FM 1960 West Suite 415, Houston TX 77069. They advised they will send a label to return the braces. WE NEVER ORDERED THEM, THE DR DID NOT ORDER THEM.THEY ARE SCAMMING MEDICARE. PLEASE HAVE THEM STOP OR ADVISE ME HOW TO GET THEM TO STOP. MY MOM IS 88 YEARS OLD AND CANCELLING THE PHONE IS NOT AN OPTION.

I received a call at my Florida home last spring from someone claiming to be a Medicare representative. She knew I had a recent shoulder surgery. She suggested I needed a back brace. Mid way through the call, she passed me on to another 'representative' who may not have been in the US... I realized this was a scam. I told them it was a scam. and hung up. I called Medicare and reported the scam phone call.

In July, when I was up North for the summer, a large package was delivered to my Florida address. My caretaker took it into my house. I returned to Florida last week and opened a large box containing a back brace and wrist guard from Prospect Health Solutions. Inc.

At the same time, I opened my Florida Blue Insurance Statement that had been delayed in delivery... Two charges to Medicare were listed from Prospect Health Solutions. $1,993.00 for LSO SC R AMT/POS PNL PRE CST & $698.00 for WHO NONTORSION JNTS PRE OTS. The total approved by Medicare was $2,182.79 !!

I checked the Better Business Bureau fraud website and saw that there had been many complaints about Prospect Health Solutions - unwanted, unprescribed medical equipment delivered to homes.

I called Medicare Fraud yesterday and reported the incident. They told me to return the package. They have directed the issue to their Fraud Department and said there have been a number of complaints about unwanted medical equipment deliveries. I paid $40 to return the package this morning to Prospect Health Solutions, Inc. 5460 N.State Rd. 7 Unit. 219 Fort Lauderdale FL 33319. Phone 954 289 0288. Thank you for your help in tracking down these fraudulent operators. They should go to jail.

Received several telephone calls early October from an unidentified party (1-904-680-0103) requesting medical insurance information required by an unidentified primary care physician. I refused to provide any information. I assume the product received below was related to these calls soliciting additional information.

On Thursday, 17 October 2024, I received a package containing a wrist brace, -L3916 Comfortland Premium Wrist/Hand Splint. I did not order this product. The supplier was AVA Med Supply, 321 Northlake Blvd, Suite #116, North Palm Beach, FL 33408, Order # AVA-46695. The bill to address was Delta Medical Group Inc., 7324 Southwest FWY, Ste 820, Houston, TX 77074, Invoice # 46695.

I notified MEDICARE of the potential fraud/scam. I was advised to return the product to AVA Med Supply via returned receipt requested postal mail.

Husband had knee replacement 2 weeks ago. 2 braces arrived 3 days ago by FedEx. Called dr to confirm he hadn't ordered- he had not. Called all numbers on forms- delta medical group- Houston tx and ava med supple in north palm beach. No answer -left messages- no call back

They called my home number several times a day for several weeks. Some how they knew all of my info. I kept telling them that if I had any pain I would notify my personal doctor. They would not take that for an answer, I even hung up the phone several times and they would call right back ( at least 5 times). I said that I didn't want any of their products. What do you know, Saturday 6/22 I get a box from UPS which I plan on returning. I think that because I have Medicare, they are trying to charge them.

Received a phone call during Memorial Day Weekend wanting a confirmation on information from Medicare. They had all my info and it was correct. On the 7th of June I received a package with 2 knee braces that I did not order. The shipping label was from AVA Medical Supply in North Palm Beach Fl. Their number is 561-880-4394. The billing address was to a company Delta Medical Group Inc, 7324 Southwest Fwy Ste 820, Houston, Tx 77074. I have since found out that this is a scam to charge Medicare. I have no idea what to do with the braces.
