I was trying to get my Small Business a UEI with SAM.gov, however, I wasn't paying attention to the fact that this was a third party website intimidating the actual SAM application, which is free through the US government. We filed everything from our address to our personal banking information. Please be cautious about this website, it looks very official and has indicators that make it seem to be a legitimate website (little errors, professional design, lengthy application) We lost $10 on a public notary, which luckily did not go to the scammer.
