A man called stating he was from the US Department of Labor. He stated he had not received the forms I had told him before that I would be mailing to him. He said he urgently needed to speak with me or he would send a subpoena for said forms.

I have never had any dealings with the US Department of Labor or had any previous contact with them. Any forms required by them for my employment would be handled through my employer's HR department.
