The other side of the card has the business name, address, toll free number and another website to order more today (covidsmellcheck.com) The description says "What is Smell Check? The quick and consistent way to screen for loss of smell. Why use it? Regularly checking your sense of smell can mean isolating quicker thus slowing the spread of infection and saving lives."

So I Googled the 800 number which doesn't come up as a valid business phone. The address is for an apartment complex in Seattle. Not sure what is really on the card if anything but quite concerning. No one needs to pay money to get a card with smells on it to test your sense of smell.
