recieved a doctor bill for a drug screen from *** *** *** - never been to this doctor and never heard of him. upon research, I found SUN-YOUNG &partner DOUG YOUNG address 1400 MACK HATCHER LANE, COLUMBIA, TN. 38101-1798 , DEPT #07-086 , ADD'L PH # 731-828-3760 , 615-828-3760. AND NPI # 1871733667 . Bill for service dated 12/17/2015 for $360.00 due for Paul M Graves (my husb). acct # MU3103 CPT CODE GO431. Is printed on authentic looking statement. When calling - there is only automated answering service stating "operator not available" pls leave message. Again, we know this is a scam and VERY concerned that this party may have add'l personal info.
