Do not use this site!! They will take your personal information and fill out a form and send it to you for $54. They do not provide a service but they are probably participating in identity theft. The site needs to be shut down. You can fill out an application online for your state if you need to get a copy of your Birth Certificate. They also have my credit card information as well.
