Thank you very much for your inquiry.
First, I want to acknowledge that, unfortunately, our services are on the costly side, with details below, near the bottom of this email. Insurance does not cover the cost of our fee, although it usually covers the cost of testing we obtain or recommend. We are acutely aware that this presents an obstacle for many people. We are working to find a practical solution to that problem, given the labor-intensive nature of our work, but regrettably we have not found one yet.
Below is some background information on our process based on how we have worked in the past, prior to the pandemic, and an update on how we are working, remotely, at this time.
The first step would be for you to send us, at this email address, some more information about your medical problem, including more information about your symptoms, what kinds of doctors you have seen, and any unexplained abnormalities on tests or physical examination. Most prospective patients send us between a couple of paragraphs and a couple of pages. The purpose of the summary is to help us understand if we are indeed a good fit for the problem. If we mutually think it looks like a good fit I will be happy to speak with you on the phone or by Zoom, and then provide contact information for some patients who have worked with us.
Prior to the pandemic, after detailed record review, we saw all patients in person for a comprehensive history and physical compressed into 3 consecutive days. During the pandemic, we shifted to a model in which, after reviewing the full medical record, we conduct our extensive history and physical remotely via Zoom. We are continuing this remote model for the foreseeable future because it has proven very effective. It has also been popular with our patients because it alleviates the need for travel, and allows more flexible scheduling of sessions over a longer period of time. We can usually begin sessions with a new patient within 2 - 4 weeks after receiving the medical records.
The in-depth history is the same as we have traditionally done in person. We can also make visual observations and do a remote, guided examination, although of course it is not directly "hands-on". If we think it would be helpful, we can then arrange an in-person visit with a full physical examination.
We then develop and refine an extensive differential diagnosis across all specialties and go through cycles of research, testing, and rethinking over an extended period. While our emphasis is on the history in particular, as well as the physical examination and conventional labs, we do deploy cutting-edge molecular genetic studies where appropriate. After the initial history and physical sessions, we continue to work closely with the patient through frequent phone, text, email and Zoom interactions.
We most often work on a flat-fee basis for up to a year of work. As you can imagine, a great deal of time and focus is involved-- scores to sometimes hundreds of hours of work-- so our flat fee for a year is in the range of ten to fifteen thousand dollars, depending on the nature of the case. Because we work on a "project" basis rather than an "office visit" basis, insurance does not cover our fee but may cover the costs of tests we recommend, depending on the details of your coverage. Eventually we hope to start a philanthropic fund to expand access to our services, but we have not reached that point yet.

I was unable to find some answers on your website. Would you mind answering the following?
What is the flat fee amount, covered for 1 year, if one is accepted?
Proteomics and metabolomics are quite expensive. Is that through a grant or out of pocket for a patient whose insurance does not cover genetics? If so, what would be the cost for each?
Do you have any data on how many cases have been solved out of your total accepted applicants?
