Type of a scam: Fake Invoice/Supplier Bill

Second time we've been contacted by this company in this way. It appears as a legit invoice due, but works like "Quote" are within the document. It appears a balance of $576.58 is due again this time just as before.

Business "Order Form" sent to our accounts payable department through the USPS. Shows to "include your sales ID number# on your check/order or detach the portion below." "Make all checks payable to Nationwide Chemicals".

This looks like a payable amount owed to their company. Company information listed is similar to ours, address is ours, phone number is another company similar to ours in another state. Company I contacted stated they did not request a quote or product either.

Again (second time) when I contacted Nationwide Chemicals by phone, they stated this was an error and we should just throw it away. Same comment as before. However, this "invoice" looks a little different. Combination of an ORDER, QUOTE, and INVOICE

Be careful! We've heard reports from several other businesses in our industry getting the same "wrong" document in the mail.

Mailed us an invoice for cleaning chemicals we never purchased. This is the second time we've received a fake invoice from this company.
