I am a nurse practitioner who received disability paperwork on a patient. The patient claimed that the company had reached out to her telling her she could get disability benefits. She reported that she answered questions but declined to pursue the disability. She denied that the signature on the paperwork was hers. The company called and faxed and mailed documents to my office multiple times trying to get paperwork completed in order to file disability benefits in the patient's name. The patient stated that she was not trying to claim disability benefits.
