- Johnstown, PA, USA • Dec 11, 2024

It looks as if my insurance information has been breached. I received an EOB from my insurance company stating that I received services from GAZAR RECOVERY INC. on 11/06/24. I live in another state so I never was at this location and never received any Orthotics. Total Charge was $9,500 and the insurance paid $2,565.44. Have not been billed from Gazar Recovery as of yet. I will be filing a fraud claim with the insurance.

I received my Medicare EOB on December 9, 2024. I immediately saw a fraudulent charge from Absolute Medical Supplies for Ostomy pouches ordered from a doctor i have never seen. Of course, I have no medical condition that warrants the use of Ostomy pouches. These charges were submitted to Medicare between February 9- June 10, 2024. This is clearly a Medicare Scam. Medicare paid the claim. I immediately alerted Medcare. They removed the charges from my account and said they would investigate. I then called the doctor's office, a very prestigious university hospital. They were not aware of this scam, but did say they would investigate and get in touch with Medicare. I can only hope this leads to the closing of this business and prosecution. They are stealing from the federal government.

Company charged Medicare and secondary insurance $19900.00 for urinary catheters and supplies that were never ordered, needed or received

- Phoenix, AZ, USA

This is through my Medicare account. They have billed my Medicare account for monthly Urinary catheter services that I have not received or need. Medicare has paid, per my EOB, $8034 for services May - Oct 2023. I have notified the Medicare fraud department about this, and they have issued me a new Medicare account. My Medicare was also billed by other companies for Covid test kits that I didn't order or receive in 2023. I also reported those cases as well to Medicare Fraud department. I saw this on ScamPulse.com and wanted to let you know as well. I am just one individual, so I am guessing Medicare is being defrauded for lots of money, which also affects all of us.

- Flasher, ND, USA

This company has charged $11,940.00 in urinary catheters to my Medicare account.

- Middle Village, NY, USA

Fraudulent claim on Medicare for 800 urinary catheters which were never ordered. Reported to Medicare as fraud and abuse in December 2023. Medicare was charged $7760 and paid $5832.96. There is an ongoing scam with G&I Ortho supply in Brooklyn as well. Reported on CBS news. Please check Medicare summary notices since these supply companies are using stolen Medicare numbers.

- Lebanon, PA, USA

I received my Medicare Summary and they charged for 3 200 Intermittent urinary catheters; curved tip with or without coating (A4352-KK) $1990.00 each

I reported it to the Medicare Fraud Line and they filed a claim.

Bill submitted to Medicare for services I did not receive and ordered by a doctor I do not know

- Denver, CO, USA

This company billed my Medicare account for incontinence supplies for 4 months -- $1990 each, total $7960 - from July 2023 thru Oct 2023. It's fraud, neither me nor my doctors ordered such supplies. My Medicare secondary caught it, they denied payment, and I saw it on their monthly report. But MC apparently DID pay them. I reported this to MC.

- Fortville, IN, USA

My Medicare Part B was charged. I never used them for any service. Medicare did not approve the charges and stated that I should not be billed for this. I did not pay them anything. The order supposedly came from Dr. Adham A Sayed-Ali. Not my doctor.

- Ramsey, NJ, USA

Medicare fraud. DME filled for. I don’t know this company or the person that ordered the DME, Rounak Ahmed! Complete FRAUD and Medicare paid it!!!!! Don’t have any association with them. Never requested DME, never received DME, COMPLETE FRAUD. I am a physician and and I detest this abuse of Medicare.

- Sparta, TN, USA

On 11/17 I received a letter stating that my personal information including my Medicare identifier number was part of a security vulnerability or hack. Yesterday I received a Medicare Summary Notice which had a claim filed by G&i Ortho Supply for $7690. Claim was denied.

- Closter, NJ, USA

Medicare contacted me about a bill from G&i Ortho that they paid. I had no contact with that company or Dr. Richard L. Addes that allegedly order the urinary cathethers that G&i billed to Medicare.

- Moscow Mills, MO, USA

A Medicare claim of $5970.00 was submitted by Dr. Tariq Waseem to G&I Ortho Supply Inc, Brooklyn, NY for 600 intermittent urinary catheters (3 claims filed) between July 21, 2023 to Sept. 20, 2023. Medicare approved and paid their portion. The remainder of balance was sent to my secondary insurance company. All of this happened before I received my summary statement December 1, 2023. I contacted Medicare fraud department and made my report to them. Medicare said this will take a long time to be removed from my Medicare account, but it will eventually be removed. Now I am trying to get in touch with my secondary insurance.

- Aurora, MO, USA

They have charged me for things that I do not need.  Like a urinary cath with insertion.  I do not need these things.  They are charging my part b medical insurance.  The government needs to check them out.  A lot of people need to check their notices from Medicare summary notice for part B medical insurance.  This is a scam.

- Murfreesboro, TN, USA
