Letter looks to be for filing my Business Entity Report. However I had just filed that a couple of days earlier and that was only $32 dollars and this one is $399.89. When I look at the two letters and compare them they are similar. Had I not just done this I might have mistaken it for what I needed to do. They even used a colored Indiana logo at the top. I could see small business owners falling for this. In the top right corner in very light gray ink and small print it does state that you can buy this from another site for less money, so maybe that is their loop-hole.

Letter to ****************, Inc. charging for filing a Business Entity Report. Charging $399.89 for a report that had already been filed for $20. Light print in the upper corner says the request is not connected to any government agency.
