Company contacted patient multiple times to tell her that she needed to send in her prescription for diabetic supplies. Then, company contacted doctor's office by fax, phone and email. Prescription was sent to office with doctor's and patient's names on it requesting diabetic supplies and medications that the patient and doctor had not requested. The items were such that they would not have been covered by insurance and the patient would've ended up with a hefty bill had we filled out the prescription form as requested.

The form was similar to many we receive from legitimate pharmacies requesting refills on behalf of their customers and had quite a bit of the patient's PII on it.
