They requested a payment by credit card in full of half the amount today.
They also sated that multiple letters were sent to my address already. When I asked who the provider that requested bloodwork was, the lady on the phone said that they only had provider number and not provider name. I asked for more information, and they followed by email with my personal information and REFERRED PROVIDER: M611333-49. This appears to be a scam. Please investigate.

I received a call from Consumer Collections 0n September 3, 2020, the lady said her name was Kimberly, she stated that I owed US Diagnostic 378.00 dollars for test I had done on April 3, 2018, I told told her I had no recollection of any type of Lab work or medical visits during that time frame, she then preceded to tell me that she would put the account on hold until her supervisor reviewed my case. They almost had me if it wasn't for me researching this company, these [censored]s need to be caught and prosecuted. The corporate office number she gave me was 1-909-358-6165 based in California. Don't fall for it people.

Received a call stating I owed $489 from service on 4/10/18 by US Diagnostic. They must always use 2018! Because I have extensive medical visits each year, I keep track of everything. I asked for something in writing showing what was done and Jackie said "We're not going to send anything else because we have sent several letters for payment to your address" (which she knew). I told her I'd call her back and did my research. First called my Dr''s offices and they all confirmed they don't use US Diagnostics. I then found a company with the same name and called them. They are the ones who told me it is a scam. I left a message for Jackie and am awaiting her callback.
